I hereby release Parkway Hoops and/or other affiliated parties from any claims or causes of action that result from any harm or injuries suffered or illnesses contracted during all basketball training sessions, drills, fitness and/or camps. I hereby waive any right that I or the above-named participant may otherwise have to bring a legal action against Parkway Hoops Training, and any appointed coaches, administrators, and/or other affiliated parties for personal injury. Because physical exercise can be subject to risk of serious injury and/or exposure to novel coronavirus, COVID-19, I knowingly assume all risks associated with participation in basketball training with Parkway Hoops Training, including harm or injuries that result from the negligence of the participants or others involved with the program, including Parkway Hoops Training, and and/or other affiliated parties. In recognition that physical exercise can be strenuous, I verify my child/ren is in good physical health and has no restrictions on his or her ability to participate in any basketball training activity. I acknowledge that I have carefully read this “waiver and release” and fully understand that it is a release of liability. I expressly agree to release and discharge Parkway Hoops Training and/or other affiliated parties from any claims or causes of action or any other right to bring a legal action for personal injury or property damage. I further agree that Parkway Hoops Training has my permission to use my child’s photograph or video publicly to promote a basketball program. I understand that the images or videos may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me or my child/children by reason of such use.
